The death certificate of Chadwick Boseman has been released, and it shares insight into the final resting place of the star.

The actor died from multiple organ failure at his home in Los Angeles on Aug. 28, with colon cancer being the underlying cause, according to the Associated Press which acquired the certificate. He was laid to rest on Sept. 3 in Belton, South Carolina, near the town of Anderson where he was born in 1976, at the Welfare Baptist Church Cemetery.

In 2016, Boseman was diagnosed with stage three colon cancer, which he kept private from nearly everyone outside his family, while shooting 10 of his biggest movie, including Black Panther, Marshall, and his final for Netflix Ma Rainey’s Black Bottom.

The report also verified Boseman underwent surgery shortly after his diagnosis to remove the colon cancer. His cancer had metastasized by March of this year which led the actor to undergo laparoscopic surgery.
